Medication Consent Form – The Medication Consent Form must be completed fully by a doctor. The parent is required to review and then sign it once the doctor has completed it. The sections #31-35 don’t need to be filled in.
The information in the form must be in line with the information found on the label for the medication. Finally, the parent should sign the bottom on the form. It is important to be aware that the information entered in the form of consent form should be reviewed to assure it is accurate.
